Target Moves Up & Down When Eyes Are Covered One After Another?

Mr Ng (M/43) came for eye examination to make a new pair of multifocal glasses. Vision with existing glasses of 4years is fine seeing far, but seeing near is blurry and he removes glasses to see phone. Slight AHP of head tilt to Right is observed.

Vision with existing glasses:
R: -3.00/-1.00×180 (6/7.5)
L: -3.50/-0.50×10 (6/6-)

Examination showed:
R: -3.25/-1.00×180 (6/6)
L: -3.50/-0.75×10 (6/6) 1 BI, 1 BD
Add: +1.25 (N5@40cm)

Other assessments showed:

Examination of Eye Alignment/Vergence (Cover Test) by looking at a target alphabet
almost NMD @Distance & Near *but he told us the target goes up & down and prompted us to check further
(normal value: Orthophoria/NMD-No Movement Detected or small XP small recovery)
EP = Esophoria is misalignment of the eye, eye is deviated inwards
XP = Exophoria is misalignment of the eye, eye is deviated outwards

Examination of Eye Alignment (Corneal Light Reflection Test – Hirschberg)
LE corneal reflex displaces nasally

Examination of Eye Alignment (Maddox Rod)
Distance : 1 BI, LE 1BD
Near : Orthophoria
(prism: BI=base in, BD=base down)

Examination of Eye Alignment (Howell Card)
Distance : 1 EP – 0
Near : 4 – 6 XP
(normal value: 1 EP -3 XP @distance, 0-6 XP @near)

3D Vision (Random Dot 3S)
25″
(normal value: 12.5”)

Mr Ng has slight misalignment in his left eye, in which we tried with prism on top of power correction. We let him compare having the prism & without the prism, he commented that his vision is much better with prism. He also told us that he always needs to put screen brightness to the max in order to see clearly. He further shared that he had numerous red eye for the past 2-3years.

Considering his significant past eye history, we would refer him to see an eye doctor/ophthalmologist first to check on the eye alignment.
