Persistent Headache? Is It Related to Glasses?

Mr Chok came with complaint of persistent headache since 1 month ago. He had gone for medical check and all results came back negative.

Mr Chok has three previous pair of multifocal glasses. He did not have any issue with two previous pairs of glasses. The latest pair was made few months back and he only started wearing it over a month now.

Latest pair:
R: +0.25/-0.25×90
L: +0.50/-0.25×90
Addition: +1.75

Previous pair:
R: +0.50/-0.50×90
L: +0.25/-0.50×70
Addition: +1.50

Oldest pair:
R: +0.25/-0.50×85
L: +0.25/-0.50×85
Addition: +1.50

Examination of eye alignment (by Howell Card)
Far vision
Latest pair : 0-2XP
Previous pair : 0-2XP
Oldest pair : 0-2XP

Near vision
Latest pair : 10-12XP
Previous pair : 6XP
Oldest pair : 6XP

XP = Exophoria is misalignment of the eye. When the magnitude of misalignment is within 6, it is normal.

For Mr Chok’s latest pair, he has 10-12XP when seeing near. This may be causing him to have headache.

We have advised him to stop wearing his latest pair of glasses & only wear his previous pair of glasses and sees if his headache subsides. He is to come back for follow up in 1-2 weeks.
