Mr Tang (M/30) came for eye examination, vision with existing glasses is fine seeing far & near. But he sometimes will remove glasses to see near as he finds it more comfortable. Lenses have many scratches & coating has peeled off.

Vision without glasses
R: 6/30-
L: 6/45

Vision with existing glasses
R: -2.50/-0.25×60 (6/6-)
L: -3.00/-0.25×105 (6/6)

Examination shows: (Delay Fogging)
R: -1.75 (6/6) (+1.00 blur lens test 6/9)
L: -2.50 (6/6) (+1.00 blur lens test 6/9-) [L: left dominant eye] L>R
Near: L>R [adding +0.25 on right: R>L but it is not preferred adding at far vision]

(normal value: orthophoria/NMD-No Movement Detected or small XP, fast recovery)
EP = Esophoria is misalignment of the eye, eye is deviated inwards
XP = Exophoria is misalignment of the eye, eye is deviated outwards

Examination of Eye Alignment/Posture (Howell Card) by 6^ Base Down on Right eye
Distance: 0 – 2 XP (blue part) 
Near: 4 – 6 XP (blue part)
+1.00 Near: 8 – 10 XP (blue part)
(normal value: 0-1 EP or 0-3 XP @Distance; and 0-6 XP @Near)
EP = Esophoria is misalignment of the eye, eye is deviated inwards
XP = Exophoria is misalignment of the eye, eye is deviated outwards

NRA (Accommodation analysis – ability to relax accommodation) by plus lens

Mr Tang’s new power has come down. Seeing near with higher power, our eyes tend to get tired easily.. thus he would remove his glasses to see near. We update his prescription and he will be comfortable seeing near with his new glasses on. His eye posture & focusing/accommodation are good, and thats great!

Happy to see eyes with good eye systems (accommodation & vergence). You have good eyes, you are happy and we are happy too as we do not need to check for so many things.. and we do not need to worry for your eyes.. scheduling you in for numerous follow ups, and keep repeating the assessments to monitor your progress.
