Niesha (F/19) came for eye examination, vision with existing glasses is fine seeing far & near. She reported feeling sleepy after using phone, especially after long hours. She wears -1.25 & -3.25 daily contact lenses occasionally and she always wears her glasses.

Vision without glasses
R: 6/9-
L: 6/120

Vision with existing Single Vision glasses of 1+year
R: -1.25/-0.50×20 (6/6)
L: -2.75/-0.25×165 (6/6)

Examination shows: 
R: -0.75/-0.50×30 (6/6) (+1.00 blur lens test 6/9) [R: right dominant eye]
L: -2.50/-0.25×165 (6/6) (+1.00 blur lens test 6/9-)
We tried to lower -0.25 shortsightedness/myopia in right eye, but she did not like it

The contact lenses Niesha is wearing (-1.25 & -3.25) is slightly on the higher side. When we wear higher shortsigntedness/myopia, we are exhausting our focusing hence getting tired eyes on near work easily. We know that she has tired eyes from her findings of low PRA, difficult with minus lenses on accommodation facility & MEM higher than +0.75. Her eye stamina, AA is fatigued or slightly reduced too. Her eye posture at near is slightly esophoric.. and it can be relieved with added plus lens at near. We update her contact lens prescription and we will assess her AA & near posture again.

Despite wearing added plus at near, her esophoria is still not relieved.. and she keeps wanting higher shortsightedness/myopia.

When we look at near posture in her existing glasses:

Niesha may be having some Convergence Excess causing her significant esophoria.. we refer for cycloplegic refraction to put eye drop at eyes specialist clinic to temporarily relax her eye muscles.
