Madam Ang came for eye examination, with complaint of decreased far vision with new 2 floaters (no flashes) seen in right eye. She had cataract surgery in right eye about 2years ago & just cataract surgery in left eye beginning of this year. She has glaucoma & she is under eye drop medication. Previously, she had an acute glaucoma few years ago.

Vision without glasses
R: 6/21-
L: 6/21

Vision with existing multifocal glasses
R: +0.50/-2.00×65 (6/18-) [face turn to left: can see 6/12]
L: -0.50/-2.25×110 (6/18)

Examination shows: 
R: +0.75/-2.00×80 (6/15-) PHNI
L: +0.25/-2.25×80 (6/12) PHNI
Add: +2.75 (N8-@40cm) / +2.50 (N6@40cm)

Other assessments show:

Examination of Macula @ Central Vision (Amsler Chart)
R: yellowish blur patch @ the centre of Amsler
L: normal
(normal value: normal-all grid lines are straight)

Colour Vision
R: normal
L: normal
(normal value: normal)

Slit Lamp Microscope
R: mild opacification, conjunctival redness with large pupil & non-reactive to light
L: conjunctival redness with large pupil & non-reactive to light

We refer patient to see eye specialist doctor/ophthalmologist for further management & treatment.
