Cataract Too Thick?

Our neighbour Mr Chan (M/68) had cataract in both his eyes when he came for an eye examination December last year.

Power of old multifocal glasses:
R: +3.50/-2.25×55 (6/45)
L: +3.25/-2.00×70 (6/12)
Addition: +2.50 (N10@40cm)
6/45 is the third largest line on our eye testing chart and new power before cataract surgery was almost the same with his old glasses.

When he presented at our practice, his cataract in his right eye was already very densed/thick. We had advised him to go see an eye doctor/ophthalmologist for cataract surgery at once as it is no good leaving your eye blurred out for too long. Also, densed cataract may be associated with another eye condition called glaucoma. Having a densed cataract means that the natural lens inside our eye gets very thick, thus it may be so thick that it blocks the drainage in our eye. When this eye drainage is blocked, the pressure in our eye can get high that may lead to glaucoma.

Mr Chan has gone for cataract surgery for both eyes just before MCO. He came back to make a pair of multifocal after the MCO was eased.

New power:
R: +0.75/-2.00×80 (6/7.5)
L: +0.25/-1.75×75 (6/6)
Addition: +2.50 (N5@40cm)

With this new power, we were very happy that his vision was optimum. But, after a few weeks, he complained that he needs a bit time to focus to see clear at far especially when he is moving. But when he is sitting down/not moving, his far vision is good. Near vision is good as he can see up until the smallest line on print. He also reported that vision becomes clearer after blink, and this indicates dry eye. Our eyes are kept healthily moist/wet by tears, and when our tears factory does not produce enough tears to wet our cornea(front part of our eye), our cornea will be dry hence seeing blurry. But when we blink, we spread tears across our cornea, cornea is moist enough, we see good vision. He is prescribed non-preservative artificial tears to moist his eyes and we let him continue wearing his glasses for adaptation.

When we followed up with him, he still had the same complaint. We double checked his power and found that his power has changed.

R: +2.00/-3.00×80 (6/7.5)
L: +1.50/-2.50×75 (6/6)
Addition: +2.50 (N5@40cm)
As his eyes were blurred out for too long, his eye may need longer time to have its true power shown. He also mentioned that his eye doctor has advised him to wait for some time before getting a new pair of glasses. We have changed to the new power and we will follow up with him again soon.

On Amsler Grid, his right eye sees a bit of crooked/wavy lines and this may be associated with the integrity of his macula (macula is the back part of our eye that is in charge of our centre vision). We have advised him to check with his eye doctor when he goes back for follow-up. In the meantime, we prescribe him Amsler Grid to do it at home. In event if he notices more of those crooked lines, he can let us know and we will prepare referral letter for him.
