Extreme Dry Eye Due to Contact Lens Wear

Ms Jojo came back for weekly follow up.

She said she has used artificial tears continuously and she almost curi-curi wanted to wear contact lens. As she finds wearing glasses difficult and so inconvenient, she hopes to recover soonest & back to wearing contact lenses. She still feels her eyes are tired, especially left eye and she reported vision is better & more comfortable right after using artificial tears. Now she must not wear contact lens at all as it will prevent cornea from getting oxygen needed & contact lens wear will slow healing process. Her previous pair of contact lens must not be used anymore.

Upon examination, her power:
R: -8.25/-1.25×5 (6/7.5) vision is good
L: -6.00/-3.00×180 (6/21) blurry, clearer after blink, vision is still poor

Corneal neovascularisation(abnormal growth of blood vessel onto cornea) is still there but the look of her conjunctiva(whitish part of eye) seems to be less reddish now.

She has very mild corneal distortion on right eye and significant corneal distortion on left eye. When surface of cornea is dry, the impression of cornea surface (seen by optometrist) will look distorted, we term it as corneal distortion.

On fluorescein staining (dye test) to detect for any defect on front surface of cornea, for right eye, mild punctate epithelial erosion (PEE) on mid part of cornea detected while localised staining of PEE at centre of cornea detected on left eye. (PEE is clinical name of cornea condition whereby many small tiny dots are observed on cornea; dots are all defect area on surface of cornea, thats why the dye can absorb into it, enable us to detect it)

She did not go to see an ophthalmologist/eye doctor when she had her red eye previously. So, we have advised her to go and see eye doctor since she wants to get back to wearing contact lens as soon as possible in which medicated eye drop may help to speed up healing of her eyes.

We have a referral letter ready for her to collect and bring along to her appointment with an eye doctor. Meanwhile, she will continue to generously use non-preservative artificial tears to lubricate her eyes.

We will see her again next week.
