WONG JING JIE

JJie (F/19) came for eye examination, with complaint of sometimes blurry far & near vision with existing glasses of <1year with no headache. No family history of squint eye is reported.

Vision without glasses
R: 6/45
L: 6/45

Vision with existing glasses
R: -3.25/-0.50×60 (6/6)
L: -2.75/-0.50×100 (6/6-)

Examination shows:
R: -3.50 (6/6) (+1.00 blur lens test: 6/9-) [R: right dominant eye] R>L
L: -3.25 (6/6) (+1.00 blur lens test: 6/9)

Examination of Eye Alignment/Vergence (Cover Test)
large Exophoria, slow recovery @Distance (breaking down into alternating Exotropia)
moderate Exophoria, moderate recovery @Near
(normal value: orthophoria/NMD-No Movement Detected or small XP fast recovery)
EP = Esophoria is misalignment of the eye, eye is deviated inwards
XP = Exophoria is misalignment of the eye, eye is deviated outwards

Examination of 3D Vision (Random Dot 3S)
12.5
(normal value: 12.5”)

We let JJie know that her left eye is seen drifting outwards sometimes. She said she actually knows that her left eye will drift out. She has realised it since she was in primary school.. in which she also complained about seeing double red light at that time. Over time, she learns to control her left eye from going out & bringing it back in.

Examination of Eye Alignment (Maddox Rod)
Distance: 4 Base In (with control)
Near: white vertical line is on her right & red spotlight on her left.. before red spotlight drifting to her right
(normal value: 0 – 1 EP or 0 – 3 XP @Distance & 0 – 6 XP @Near)
Base Out/EP = Esophoria is misalignment of the eye, eye is deviated inwards
Base In/XP = Exophoria is misalignment of the eye, eye is deviated outwards

Examination of Binocular Perception (Worth 4 Dots)
Distance: 5 dots seen (double vision) [with her relaxing her left eye] {4 dots seen – with control}
Near: 5 dots seen (double vision) [with her relaxing her left eye] {4 dots seen – with control}
(normal value: 4 dots = normal; 3 dots = Right suppression; 2 dots = Left suppression; 5 dots = Double vision)

W4D @Distance & Near – drawn by patient of what she sees

Fusional Reserves (Convergent/Divergent reserve stamina) by Prism Bar (with her controlling her left eye)
Near, Base-Out/Convergent, PFV: x/6/4
Near, Base-In/Divergent, NFV: x/12/10
Distance, Base-In/Divergent, NFV: x/8/6
Distance, Base-Out/Convergent, PFV: 16/25/18
(normal value: Blur/Break/Recovery)
Near, PFV: 10-28/7-21
Near, NFV: 7-19/5-15
Distance, PFV: 4-18/5-9
Distance, NFV: 4-10/2-6
[Scheiman et al – Adult Data]

Interpupillary distance, IPD = 6.6cm

We tried
R: -3.50 2Base-In
L: -3.25 2Base-In

She said that her vision is better & her left eye tends to drift out lesser, but it still drifts out

We tried
R: -4.50 2Base-In
L: -4.25 2Base-In

She said the same thing too.. her vision is better & her left eye tends to drift out lesser, but it still does drift out

We refer her to see eye specialist doctor/ophthalmologist & orthoptist.
