Mr Simon (M/60) came for eye examination, vision with existing multifocal glasses of 3-4years is slightly blurry. His left eye is deviated out (Exotropia), he has been having this since young age. He also reported that he previously had tried with prism to align his eye but to no avail by his eye care practitioner when he was younger. No history of trauma reported.
Examination shows:
R: +0.75/-0.75×5 (6/6-)
L: +1.00/-0.50×15 (6/12-)
Add: +2.75

Examination of Eye Alignment/Vergence (Cover Test)
alternating Exotropia @Distance & Near, mostly Right fixating eye
(normal value: orthophoria/NMD-No Movement Detected or small XP small recovery)
EP = Esophoria is misalignment of the eye, eye is deviated inwards
XP = Exophoria is misalignment of the eye, eye is deviated outwards
We tried with prism but it does not seem to align back the eye. And sometimes, his outward eye turn/exotropia may shift to the other eye, but mostly his fixating eye is Right eye. He also has AHP having head tilt to right and face turn to left. We noted of this AHP again during measurement taking when he insisted a certain position of his head during the process to give single vision, otherwise he may see blurry double vision.
Given the nature of his eye turn, we have let Mr Simon know about seeking orthoptist & ophthalmologist for a second opinion but he is not keen about it.
