Dim Vision With No Pain In One Eye?

Mr Chua (M/44) came for eye examination, with complaint of dim vision with no pain in his right eye since about a month ago. No recent trauma is reported.

Vision without glasses
R: 6/21- slow
L: 6/18

Subjective Refraction
R: PL/-1.00×90 (6/18- slow) PHNI Add +1.50 N16
L: PL/-1.00×75 (6/9) PHNI Add +1.50 N5
Pt reported seeing shadow/double with both eyes.. no shadow seen monocularly
Pt reported seeing green as white colour in RE (during duochrome test)

Other assessments show:

R: defect
L: normal

Cover Test
Distance: small exotropia & hypotropia (very subtle) (RE mild ptosis)
(cover LE: pt reported target goes slanting 7o’clock to 1o’clock & smaller)
(uncover LE: pt reported target goes slanting 1o’clock to 7o’clock & bigger)

Fluorescein
R: -ve
L: -ve

Pupil Reflex
R: -ve RAPD (pupil is not dilated)
L: -ve RAPD

Amsler Chart
R: black patches & distorted lines (inferior-nasal quadrant spared)
L: normal

Mr Chua has diabetes & taking medication since 8years ago.
